[efh] server troubles

You're not doing anything wrong.  Recent security upgrades in the 
Apache server cause access to directories without index files to be 
denied.  This is in case you're accidentally keeping a bunch of 
private stuff inside your web-visible directory tree.  (This happens 
all the time, particularly when people store music files in their 
accounts.  Those turn out to be web-visible, which produces a lot of 
DMCA crap we'd rather do without.)  Sorry about the added hassle of 
having to create and maintain an additional index.html page.
